folder icons = folder.jpg otherwise it shows the golden Bluray or the first jpg image in the folder
if your folder shows the golden Bluray that implies you have no JPG files in this folder.

The sheets only work in the Gallery View which is the mode with the 8 thumbnails on the bottom of the screen.
Something is wrong in your settings and nobody can know what is wrong because the description is not giving any clues. In this whole process is no "ect." 
it is crucial to have accurate file names (bottom of page 14 in the manual) and it is crucial to have the exact settings in thumbgen as on page 9 & 10 in the manual. Most of the times people are missing the "movie info" settings and the "input/output naming" settings. The important stuff is in red boxes.
